Advanced Care Planning

Advanced Care Planning is the process of thinking about and communicating your wishes, or your ‘goals of care’. As your family doctors, knowing your goals of care allows us to help you navigate your health care choices.

And in the event that you are unable to make choices for yourself, it helps us and your loved ones know what you might have chosen for yourself – so we can best support you.

But we know, these discussions and topics are difficult, try to discuss them with your loved ones.

Falls Prevention

If falling is a concern for you or a loved one, we recommend you consider exercises specifically designed to strengthen problem areas for the elderly.
