Practice Policies
- To be eligible for registration, a patient must be enrolled in MSP; and live the in the catchment area; and have a PHN.
- Late Cancellation or No Show
If you cannot make it to your appointment, you are required to cancel the appointment by giving a minimum 24 hours notice so that we can give your appointment to someone else who needs it. If you fail to cancel your appointment, you will be charged the missed appointment fee. - Please sign consent form to communicate electronically (this includes telemedicine appointments)
- Family doctors in Canada have a large patient panel. Please help us to be as efficient as possible by arriving ahead of the scheduled time and be aware of our time slots. If you have more than one issue, several appointments will need to be booked to ensure that all issues are dealt with appropriately. Please inform the staff of the reason for your visit or put reason in online booking.
- Different appointment types should not be mixed. Can not do ICBC, WCB issue, etc. with MSP concern even if prescription refills only.
- Patients with chronic diseases do better if they see their doctor regularly. Follow up visits are at least every 3 months for some chronic conditions.
- We occasionally encounter emergencies/seriously ill patients. These take priority and may delay your appointment/calls. Please be patient. Late arrivals maybe required to rebook if there are patients already waiting.
- Emergencies are best dealt with in emergency rooms – which provide laboratory and x-ray facilities. (Note-Dr Carroll also works at the Cancer clinic and so is not in the office daily)
- You can only have one family physician at a time. Multi-doctoring is dangerous and can result in your discharge from the practice.
- Cause for immediate dismissal include requests for dishonest, illegal, or deceitful activities (lying on forms, requests to back date forms, altering diagnoses or charts, shouting/rudeness at any staff, swearing, threats, demands or aggression of any kind, failure to pay bills).
- If you have any investigations, you are required to come in or follow up approximately 1 week following the test. This serves the purpose of ensuring that your tests are received and reviewed and nothing is lost.
- Certain medications are addictive. These prescriptions are governed by law and are strictly controlled as they may be traded on the black market. They require a contract to be signed for continued prescribing.
- All tests, referrals, and medications may result in side effects. A discussion is always welcomed and may help to ensure that appropriate tests, referrals, and treatments are made. Prescriptions and tests are ordered at the doctor’s discretion.
- For virtual appointments re mood or ADHD follow up – pls email updated questionnaires.
- We may have Residents and PCN nursing. You may be seen by (or in the presence of) a student with your consent. Please inform the staff if you wish to have the resident not present when you come for your visit.
- Many services are not covered by provincial health coverage and are charged to the patient. You will be charged for services such as forms, sick notes, record reviews for private reasons and record transfers, and many other uninsured services and procedures. Please make yourself familiar with the charges. Ask at the front desk if you need further information. Allow one month for completion of forms. Please be aware patients are responsible for all fees and can claim back from insurance etc. if appropriate.
- Fees are usually due at the time of the service.
Policies may change from time to time. If you have any concerns, please check with each visit.
FORMS
Due to a high volume of forms, we have had to adjust our process.
REVIEW AND COMPLETE THE STEPS BELOW.
- COMPLETE ALL of the parts of the form that YOU are to complete, including your signature for release of information and date. Forms can not be completed unless signed.
- It may be helpful for you to fill out a draft of what YOU would like included on the form.
- Drop off the ORIGINAL form to the office once this is completed.
- Our MOA will assess the form and call you to arrange an appointment and discuss any fees related to any forms and book appropriate appointments.
- Please be sure to inform us of any completion deadlines, and information on where and to whom the form is intended to go to.
- PLEASE DO NOT SEND FORMS BY EMAIL UNLESS SPECIFICALLY INVITED TO DO SO. Also, ensure you have signed consent to communicate electronically.
- Most forms fall under uninsured services and fees are the responsibility of the patient.
